Primary Care Nurse Practitioner – Hybrid Remote

Looking for an NP opportunity that offers flexibility, innovation, and the chance to make a real impact in chronic disease management? We have a few full time, permanent openings in the Essex County area.

The Role This is a primarily telehealth-based position; most of your work will be done virtually, with the occasional option to see patients in person if they’re local to you.

You’ll be part of building something new: an innovative virtual and in-person care model focused on providing primary care for patients with (CKD) and related chronic conditions.

Primary Conditions you’ll manage:

• Chronic Kidney Disease (CKD)
• Hypertension
• Diabetes
• Heart Failure

Schedule & Lifestyle

• Monday-Friday only
• Self-managed schedule
• No nights, no weekends, no on-call

Requirements:

• Minimum 5 years of NP experience (primary care, internal medicine, cardiology, nephrology, or case management)
• Experience managing chronic diseases (CKD, CHF, diabetes, hypertension)
• DEA license (or ability to obtain within 90 days)
• Current CPR/BLS certification
• Comfortable working across multiple EMR systems
• Strong computer skills – MS Word, Excel, Outlook

Preferred Qualifications

• Telehealth experience (strong plus)
• Value-Based Care or Population Health background
• Medicare Advantage risk documentation/coding experience
• Experience reducing avoidable hospitalizations through proactive, coordinated care
• Familiarity with palliative care discussions
