Home Health Registered Nurse (RN) – Case Management
Location: Passaic County, NJ (Clifton & Passaic)
Schedule: Monday-Friday, 8:00 AM-5:00 PM + Weekend/Holiday On-Call Rotation
Mileage Reimbursement: $0.50 per mile (patient-to-patient travel)

About the Role

We are seeking a compassionate and experienced Home Health Registered Nurse (RN) to provide skilled nursing care, case management, and health education in a community-based setting. This role focuses on supporting patients with chronic, acute, and preventative care needs while collaborating with an interdisciplinary care team.

Responsibilities

• Assess, plan, implement, and evaluate nursing care for well, chronic, and acutely ill patients in home or community settings.
• Provide case management and ensure coordination of care across multiple disciplines.
• Monitor patient health status and provide disease prevention and health maintenance education.
• Communicate changes in patient condition to physicians and care teams promptly.
• Manage medication processes including ordering, monitoring, storage, and documentation.
• Transcribe physician orders and complete required nursing documentation (daily, monthly, quarterly, and annual).
• Maintain infection control standards for equipment and supplies.
• Schedule and coordinate patient appointments and ensure follow-up on findings.
• Maintain communication with inpatient facilities and discharge coordinators when patients are hospitalized.
• Train and supervise LPNs and unlicensed staff on health, safety, and patient-specific care needs.
• Provide new hire orientation and annual health and safety training as needed.

Requirements

• Active Registered Nurse (RN) license.
• Current CPR certification.
• Experience with:
• • Wound care
  • Foley catheter care
  • IV infusions
  • PICC and midline management
  • Homecare Homebase (HCHB) or PointCare documentation systems
• Ability to participate in weekend/holiday on-call rotation.
• Reliable transportation and willingness to travel throughout Passaic County.
