Job description:
Patient Assessment & Care Planning:
Plan, implement, and document nursing care tailored to patient needs.
Identify patient needs and prioritize care through detailed assessments, planning, and evaluation.
Develop, implement, and regularly update individualized care plans based on ongoing patient assessments.
Initiate assessments based on patients’ clinical needs, the physician’s diagnosis, and the functional health framework.

Nursing Interventions & Care Management:
Perform therapeutic nursing interventions to meet patient care needs.
Manage the care of patients, ensuring that all interventions align with clinical standards.
Document patient care based on established standards, clinical priorities, and patient acuity.

Evidence-Based Practice:
Advocate for and support the adoption of evidence-based care standards.
Incorporate innovative practices into care delivery to ensure the highest quality patient outcomes.

Patient-Centric Care:
Provide patient-centered care in alignment with the professional nursing practice model.
Collaborate with multidisciplinary teams to ensure effective patient care and optimal outcomes.

Required Certifications & Licensure:
Associate of Science in Nursing (ASN) – Required
NIH Stroke Scale (NIHSS)
Telemetry Certification
BLS – AHA
CEU Contact Stroke (4 Hours)
Rn state NJ or compact
